Statement of Disability
One of the eligibility criteria for TRIO SSS is having a documented physical, mental, or learning disability meeting the ADA requirements. TRIO SSS needs documentation from our participants for grant records. The documentation could include an accommodation letter from an institution of higher education, the TRIO SSS Disability Verification Form, or a letter from a medical professional, but other documents can be accepted. If you have documentation of a disability or would like the TRIO SSS Disability Verification Form to complete, please email TRIO SSS (ssswebp@nwmissouri.edu).
-
Do you have a documented physical, mental, or learning disability that substantially limits one or more major life activities (defined in section 12102 of the Americans with Disabilities Act)? *
-
TRIO SSS does not provide any living or learning accommodations for Northwest students; our program provides services to students to assist them in obtaining an undergraduate degree. I understand that I must go through the Northwest Title IX & Equity Office (ada@nwmissouri.edu) to receive accommodations.
-
Have you connected with the Northwest Title IX & Equity (Accessibility & Accommodations) Office to set up any learning and living accommodations you may need? For accommodations, email ada@nwmissouri.edu. *

Would you like TRIO SSS to share your information to the Northwest Title IX & Equity (Accessibility & Accommodations) Office? *

Vocational Rehabilitation services help eligible people with disabilities obtain, maintain, or advance in employment. These services are provided by Vocational Rebahilitation (VR) programs, part of the Department of Elementary and Secondary Education. The program is supported by federal and state funds.
VR is a separate program from TRIO SSS and is NOT directly affilliated with Northwest Missouri State University, though Northwest students, faculty, and staff may be eligible for their services. *
-
I give my permission to the TRIO Student Support Services professional staff to share limited disability-related information about me with my TRIO SSS Peer mentor so that they may adequately assist me in my academic progression and success at Northwest Missouri State University. *
